Enrollment Agreement
As the child care provider, Calvary Lutheran School of Early Childhood will:
♦Give your child(ren) loving care, constant supervision, and worthwhile, stimulating, and developmentally appropriate activities. Children who are here in the mornings, except for infants and toddlers, will attend our regular preschool classes.
♦Provide a nutritious lunch, and provide two snacks daily.
♦Cooperate with parents for toilet training, guidance, and other needs.
♦Provide transportation to and from school as needed, notify parents, in advance, of scheduled field trips, and ensure each child is properly restrained in a car seat or seat belt as determined by state law.
♦Give parents two weeks notice if we feel other arrangements need to be made for your child(ren).
♦Mail an updated bill twice a month, near the first and sixteenth of each month.
As the parent, you will:
♦Submit completed enrollment forms promptly.
♦Submit accurate immunization records within 30 days of enrollment, and bring updated immunization records whenever your child receives additional immunizations.
♦Provide Calvary Lutheran School with necessary items needed for the care of your child, including diapers or pull-ups and baby wipes, a change of clothing, nap supplies, and appropriate outdoor clothing.
♦Inform the office of any change in address, telephone numbers or job of either parent.
♦Keep children at home when they are ill, and inform the office of a child's absence.
♦Immediately pick up children who become sick while at Calvary Lutheran School.
♦Pay all child care fees in full when payment is due.
♦Pay a late pick-up fee of $1.00 per minute for each minute your child is with us after our 6:00 p.m. closing time. This applies to all parents, even those whose regular day care expense is paid by HHS.
♦Inform the director and your child's teacher at least two weeks in advance if your children will be absent because of vacation, or if you intend to withdraw your child from Calvary Lutheran School of Early Childhood.
